When importing lessons, all resource forms are visible, but only one at a time should be

When importing a new lesson and adding resources, I used to choose a type of resource from the drop-down (Lesson Notes, Translations, Videos…) and that would show/expose the correct form for me to fill out.

Now all the forms for all resource types seem to be showing. I have been unable to add a YouTube link to my imports.

I see the following errors in Chrome’s Developer Tools console:
