Can you tell I’m pregnant?
Is this sentence correct? Should I put “if” before “I’m pregnant”?
So the sentence would be: Can you tell if I’m pregnant?

I think it sounds better with the if but without the if is fine too, but probably only in speech. In writing, putting the if there makes the sentence more clear.

Without the “if”, you know you are already pregnant. You’re really asking if the other person can see that you’re showing.
“Can you tell I’m pregnant? Am I showing yet?”
With “if”, you don’t know. You’re asking the other person to participate in guessing with you.
“Can you tell if I’m pregnant? Should I take a test?”
It’s not a matter of which sounds better, it’s two distinct meanings you’re trying to convey.
